Sinister Hospital is a horror adventure set in the forests of Duskwood, where the player enters an abandoned hospital while tracing a group of missing children.
The game begins with exploration and puzzles in the forest, including map assembly and symbol interpretation. Inside, dolls provide clues, trigger threats, and pursue the player through corridors. Progress involves examining and collecting objects, finding codes and keys, investigating records about Judge Harlan, bypassing an old security system, and navigating a doll-guided maze.
