As officer Jason Thompson, players investigate an abandoned hospital and its underground facilities while searching for clues about a dark mystery. Exploration involves examining surroundings, navigating claustrophobic corridors, solving environmental puzzles, and avoiding traps. An entity pursues the player throughout the facility, requiring stealth and careful movement. Players can manually hold their breath as it passes, but staying silent too long causes a gasp that may reveal their position. Limited resources affect which items can be carried and how supplies are used. When escape is impossible, weapons and the environment can be used against grotesque creatures.
